Underweight = <18.5
Normal weight = 18.5-24.9

Severly Obese = 35 – 39.9
Morbidly Obese = 40+

Before your Initial Office Consultation with your surgeon, please view the Getting Started Presentation below. Once you have reviewed the presentation, print and complete the accompanying quiz. Bring the completed quiz with you to your Initial Office Consultation and give to the front desk. If you do not have access to a computer, or have difficulty accessing the presentation or quiz, please let your Patient Navigator or a Patient Service Representative know and they can provide paper copies.
