BMI Categories:
Underweight = <18.5
Normal weight = 18.5-24.9

Severly Obese = 35 – 39.9
Morbidly Obese = 40+

Online Education

Some insurance companies require pre-operative education before surgery. 
If the Patient Navigator has notified you that you may complete this online education as part of your insurance company’s requirements, please do the following:
 
1)      View the Nutrition, Behavior Modification, and Exercise Presentations.
2)      Print and complete the corresponding quizzes.
3)      Bring the completed quizzes with you to your next office visit.
